  Housing Stabilization   


 This service has two parts that can be used to help individuals find and maintain stable housing.  Sustain aims to support the individual as they maintain their housing. Transition can be used to help a person plan and move to stable housing.

 Sustain 

  •  Sustain is intended to help individuals with the skills and tasks to maintain stable housing.  
  • Sustain is a great service to assist low income seniors with simple supports to keep them in their own home. 
  • This service offers 150 hours of support to help with weekly routine tasks such as grocery shopping, light housework, running errands or navigating community supports like visiting the local food shelf. 
  • This service can be a game changer in helping those who live independently maintain their self sufficiency to lead active, healthy lives.


Transition

  •  Transition evaluates housing wants and needs and find the available options.
  •   We help navigate the application process. 
  •  We will facilitate the move
  •  Our staff can assist with resolving housing barriers.
  •   We are committed to weekly in person check in with clients to provide reminders and resources to support long term housing stability.
  •   We support self identified goals to promote stability and independent living.
  • Our staff is focused on community engagement and relationship building skills

 To be eligible for this service one must be 18  or older, on Medical Assistance, and complete a MNchoice assessment or have a professional statement of need (PSN). The PSN is a document that is completed by a professional which outlines what supports are needed. The recipient then completes a person-centered plan that puts them in contro

 To be eligible for this service one must be 18  or older, on Medical Assistance, and complete a MNchoice assessment or have a professional statement of need (PSN). The PSN is a document that is completed by a professional which outlines what supports are needed. The recipient then completes a person-centered plan that puts them in control of how best to utilize the supports they will receive.  


Step 1. Fill out an Acorn Referral Form


 STEP 2. Get an Assessment  


  •   Qualified professional, like a mental  health professional, nurse practitioner,  or doctor 
  •  Certified assessor (with a MnCHOICES  assessment) 
  •  Senior care coordinator, or 
  • Coordinated entry assessor. 


STEP 3. Make a Person-Centered  Plan

  •  If you have a MA-Waiver case manager,  senior care coordinator, or targeted  case manager, that person will help you update your plan to include Housing  Stabilization Services.  
  •  If you don’t have any of these people  helping you, you can get help making a  Housing Focused Person-Centered Plan from a registered housing consultant.  No matter who helps you with your  person-centered plan, you get to decide  which provider will help you with housing  transition and housing sustaining services.  

STEP 4. Get Approved… 

  • Your assessment, proof of  disability, and person-centered  plan are sent to the Minnesota  Department of Human Services  (DHS) for approval.

Our Founders, Tracy Pierce and Carmen Pouliot have over 20 years of  community engagement in housing and hunger supports.  We built this business with the passion that those who receive the services we provide should be the focus and reason for what we do.  Sounds simple, but it is the idea that those who need the supports  know the best 

Our Founders, Tracy Pierce and Carmen Pouliot have over 20 years of  community engagement in housing and hunger supports.  We built this business with the passion that those who receive the services we provide should be the focus and reason for what we do.  Sounds simple, but it is the idea that those who need the supports  know the best way to get everything they need from them.  It is putting the client in the driver's seat and trusting them to make the best decision for THEMSELVES.  It is NOT telling them what to do or how to do it.  Our staff is here to support what our clients need based on their direction.  At Acorn, we believe our services have the potential to be a game changer to make a real change in the struggle of housing instability.  Our goal is to make the process easy and useful to sign up and receive.
