Services (H.S.S.)
These services are split into two parts to help residents of Minnesota, struggling with housing instability, to find and maintain stable housing.
The Transition services are used to help a person plan for, locate, and move into stable housing.
The Sustain portion aims to support the individual long-term in order to maintain their housing once they have it.

STEP 1. Fill out a Self Referral Form
(-or- complete the Do I Qualify? page.)
STEP 2. Get a PSN (HERE) and bring it to a Qualified Professional to get it completed.
STEP 3. Get help making a
Housing-Focused Person-Centered Plan
*Either way: no matter who helps you with your Person-Centered Plan, you personally still get to decide which provider will help you with your housing transition and housing sustain services.*
STEP 4. Wait to get Approved…

** To be eligible for this service you must reside in the state of MN, be 18 years or older, be on Medical Assistance, and have a disabling condition verified by a Professional Statement of Need (PSN). The recipient then completes a Person-Centered Plan, which puts them in control of how best to utilize the supports that they will receive. **
